FIND clients with or at risk for under or malnutrition
FEED clients the appropriate oral nutrition supplement to help improve outcomes
FOLLOW clients' progress to encourage adherence
By becoming a part of Feed the 485, your agency will have access to the education, tools, and resources you need to implement the following:
FIND:
Integrate a nutrition screen into your start of care assessment/OASIS (if not already included)
Educate your team on the impact of nutrition can have on patient outcomes and how to complete the nutrition screen
FEED:
Implement a nutrition protocol/decision tree to help your staff identify the appropriate nutrition intervention
Incorporate the nutrition recommendation into the 485/care plan
Educate patients and caregivers on the importance of good nutrition with education materials, product samples and coupons
Train your team on basic nutrition therapy and oral nutrition supplements

Foster success through tools and tips to help incorporate nutrition changes into the daily routine
